INTERACT® Certified Network

Organizations that are included within a post-acute care network will embrace the INTERACT QIP as a standard of practice for reducing unnecessary hospitalizations and achieving positive clinical outcomes. The organizations within the network will follow the below criteria:

Maintain Certification as a Certified INTERACT Facility (CIF criteria below):

  • Certification is good for two years
  • Must have two entity INTERACT Certified Champions (Primary and back-up) at all times
  • All Facility Managers and Leaders – required to attend Leadership training on INTERACT® and how to support the program within the organization
  • Onsite training for all levels of the organization on INTERACT QIP, P&P as well as their respective roles and responsibilities. INTERACT® is included in the orientation program for all staff and annual staff development
  • Medical Director INTERACT® orientation with facility leadership
  • Medical team orientation on INTERACT® and expectations from the organization
  • Approval of the program from the facility Medical Director
  • Policy and Procedures that are in alignment with INTERACT QIP
  • Data and metrics – aligned with INTERACT® program
  • INTERACT Certified Champion and organization will have access to INTERACT® Master Trainers and information portal to assist with questions, implementation strategies and updates as indicated.
  • Follow Up:
    • The facility will set reviews consistent with the Certified INTERACT Facility requirements
      • Data Review
      • Interviews with the INTERACT Certified 4.0 Champions, Administrator, DON and frontline staff
      • Chart Reviews (including use of INTERACT forms and resources)
      • QI Review Tool Reviews and Data Review (re-hospitalization rates)
      • Action Plan developed and provided to facility, based on review

 

Certified Network additional considerations:

  • Organizations that are included within the Network will adhere to Network protocols as well as embrace the full implementation of INTERACT QIP within each member organization
  • Certification is good for 2 years
  • Facilities in the network must have completed the Certified INTERACT Facility requirements, have implemented the INTERACT 4.0 Quality Improvement Program in the facility and be in good standing
  • Data and metrics
    • Network will utilize standardized readmission data collection
    • Facilities must submit re-hospitalization data to the network on a monthly basis
    • QI review of data to be completed on a monthly basis with the Network lead to determine opportunities for improvement
  • Quarterly web-based network calls for questions/support with Network Leaders and assigned CIMT.
  • Threshold to be determined by the Network on Re-Hospitalization Rates
    • Required education and consultation if threshold not met
    • Date for compliance
