    Certified INTERACT Champion 4.0 (CIC) 

    Purpose

    The CIC training is designed to train clinical leaders who will serve as champions and co-champions to implement and sustain the INTERACT Quality Improvement Program (QIP) in their organization through education, embedding the INTERACT QIP into routing clinical care and QI processes, monitoring INTERACT QIP processes and outcomes, and providing the facility team and leadership with continuous feedback on INTERACT (QIP) performance.

    CIC Course Description:

    In this two-day interactive course, the CIC Master Trainer will provide an in depth description of the INTERACT QIP strategies, care processes, tools, and other resources to improve care of changes in condition and prevent hospital transfers when safe and feasible; and share key lessons learned on successful Program implementation and sustaining the Program over time.

    Learning Objectives:

    Upon the completion of this training session, the attendee will be able to:

    1. Describe the current landscape of health care reform and funding that make the INTERACT an essential QI initiative for post-acute and long-term care organizations
    2. Articulate the key strategies that form the foundation of the INTERACT QIP
    3. Understand how to optimally utilize INTERACT QIP tools and resources
    4. Define key strategies for successful INTERACT QIP implementation, including how INTERACT can help meet QAPI requirements
    5. Define key strategies for successfully sustaining the INTERACT QIP implementation processes

    Initial Certification

    1. Complete the initial two-day training

    Certified Home Health INTERACT Champion (CHHIC)

    Purpose

    The CHHIC training is designed to train clinical leaders who will serve as champions and co-champions to implement and sustain the INTERACT Quality Improvement Program (QIP) within their Agency.

    CHHIC Course Description:

    In this 2-day interactive course, the CHHIC Master Trainer will provide an in depth description of the INTERACT QIP strategies, care processes, tools, and other resources to improve care of changes in condition and prevent hospital transfers when safe and feasible; and share key lessons learned on successful Program implementation and sustaining the Program over time.

    INTERACT® for Long Term Care Leaders Target Audience: Skilled Nursing, Assisted Living and Home Care

    Course Description:

    This 1-day interactive program will provide facility leadership with an introduction, background and overview of the purpose for the INTERACT QIP Program. An overview of the entire INTERACT(QIP), including the Tools and Resources will allow facility leaders an understanding of the process for oversight, strategies for effective implementation and how the process fits into QAPI to reduce unnecessary hospitalizations as well as an understanding of the process in order to support the Certified INTERACT Champion in the organization. This presentation will also provide a description and overview of the INTERACT QIP strategies, care processes, tools and resources that represent an evidence-based standard of practice along with description of the employee training and implementation steps necessary to sustain the program overtime for success. Facility Leadership will gain keen insight on the use and implementation of the INTERACT QIP for competitive positioning in the marketplace. Operational and clinical leaders will be provided with key leadership strategies to ensure positive clinical, quality and operational outcomes.

  • Two-Hour Leadership Program: Creating the Foundation: Leadership Strategies Operationalizing INTERACT (QIP)

    Course Description:

    An overview of INTERACT® and Operational Strategies. Understanding and implementing INTERACT Quality Improvement Program (QIP) is critical to post-acute care organizations as they prepare for the future of health care. Bundled payment, Medicare share programs, payment reform, Accountable Care Organizations, and integrated health networks are formulating across the nation at a rapid pace. Post-acute care leaders must align their organization with other health care providers in order to meet the demands of the new health care environment-Value Based Purchasing. The first step in the implementation of VBP is the reduction of unnecessary readmissions to the acute care setting. Preventing unnecessary hospital readmissions is essential for quality, compliance and now, will be a fiscally crucial step with Value Based Purchasing and the Readmission Measures.
